I utilize Valant electronic medical records system for my clients. When we set an appointment, you will receive an email from Valant with directions for setting up a patient portal account so you can provide demographic information and complete questionnaires. It is helpful for me to be able to review those prior to our initial session. If you are not comfortable using a web-based program, you can complete forms when you come for your appointment.
On Valant you are able to see when you next appointment is scheduled. Please come to your initial appointment 15 minutes early to complete paperwork.
